COVID-19 Collaboration: Neonatal/Pediatric
Note: In order to avoid having to update two sources constantly, this presents just information that is noted to be notable. General information about infection control, etc., can be found on the main COVID-19 summary page.
Reminder: We generally make changes every few days to this page, but try to save this log to the more important changes we want to highlight (to respect your time!).
5/24
- Added a few brief points from new (5/20) CDC update on Caring for Newborns with COVID
5/22
- Added MGH FLARE on pediatric pathologic inflammatory response (similar to Kawasaki Shock Syndrome), severe cases which result in shock, respiratory failure, the need for VA-ECMO. Treatment has thusfar been modeled after Kawasaki approaches (IV immunoglobulins, aspirin, steroids). Presentation is typically: Fever (3 or more days/persistent, and at least 2: rash or conjunctivitis, hypotension/shock, myoacardial dysfunction, coagulopathy, GI symptoms, as well as elevated markers of inflammation with evidence of COVID-19 or contact with COVID-19.
5/18
- Added multiple literature links, including MGH FLARE on peri- and postpartum care, Coronavirus Infections in Children (Zimmerman et al)
5/15
- Added CDC health advisory (5/14) regarding hyperinflammatory Kawasaki presentation
5/14
- Added some brief discussion around current thoughts on hyperinflammatory state - similar to Kawasaki Disease presentation as there have been reports of an increase in cases recently. This would be post-infection, and with most children asymptomatic during presentation (and thus not tested), it is a difficult correlation to establish.
5/4
- Incorporated considerations from Ong et al (PCCM) "Coronavirus Disease 2019 in Critically Ill Children: A Narrative Review of the Literature"
4/30
- Added Neo/Ped Included Guidelines/Statements. The goal of this section is to provide easy-access to most major guidance (which is certainly more limited for neo/peds currently)
- Incorporated documents from PedsAnesthesia.org, "Preliminary Observations" that incorporates thinking from Gattinoni to vent strategies for children.
- Have gone through, systematically, and reviewed all current resources, made many smaller changes. Added quite a bit of guidance in the vent section.
Our Goal is to be a Summary of Available, Reputable Advice. We review the material below on a regular basis (at least weekly, though major sources like ACCP, SCCM, AHA, etc., are reviewed nearly daily). Links to major COVID areas are provided when available. Note that usually our in-line citations are links directly to the source we summarized. Also note, we present this information to the best of our knowledge, generally within the scope of our practice (respiratory therapy) - please ensure you review and critically consider all sources presented.

| Clinical Progression |
While moderate-to-severe disease is reportedly less than that in the adult populations, there are case studies suggesting illness and death. This seems to correlate some to age - the older the child/teen, the more likely (though still rare) to end up critically ill.

| Noninvasive Ventilation |
As with adults, there is some disagreement around the safe use of noninvasive techniques such as HFNC, bubble CPAP, N-CPAP, CPAP, NPPV (bilevel or BiPAP) If at any point an FIO2 of 60% or greater is needed to keep O2 within range and/or an increased inspiratory effort (WOB) is noted, consider the need for intubation
|
||||||
| Invasive Ventilation |
Note: as with adults, there has been some evidence that children may present with either hypoxia despite near-normal compliance (non-ARDS, hypoxic vasoconstriction) vs hypoxia with poor compliance (likely meeting criteria for ARDS). Most consensus statements for COVID-19 recommend lung-protective strategies (ARDS) Strategies

Micropractices are things you can do during a workshift to manage your well-being. Being intentional with your own wellness can be difficult to do when things are overwhelming, when you're being swallowed by everything happening. That's why it is all that much more important. Just like there are no emergencies in a disaster (we take the time to properly gear up, avoid exposures), there are no reasons not to apply micropractices to emotional well-being. - Private Facebook Group for PTSD Support, specifically for RTs
